Re-stocking Asset reserve

I'm wondering if the asset reserve should always be immediately re-stocked anytime an asset is taken from the reserve for any reason. Or is it only re-stocked in connection with an Acquire Assets action? To put it another way, is it possible that there can be blank slots in the reserve, or is this something that can never happen ? Couldn't find a specific rule addressing this..

(I realize that the "Faded from Society" rumor can potentially remove all assets from the game eventually. But as an example, if that card effect removed an asset from the reserve, is it then immediately replaced from the asset deck, or does that slot remain blank?)

You omitted an important sentence from the quote. The way you have it sounds as though you immediately replace each card as it is drawn. However, the sentence after the one you quote says "During an Acquire Assets action, the active investigator does not replace Asset cards until the action has been completely resolved."

Yeah, so if I roll three successes, and pull a 2 Card, and it's immediately replaced by a 1 card, I can't also pick that up - my action is over. However if there are both a 2 card and a 1 card lying on the board at the same time, i could pick those both up, and they would be replaced by cards from the deck at the end of my action.

Correct - the rule is there to ensure that you can only buy the Assets that are present when you roll your Influence Test

Related to re-stocking the asset reserve, there is a question raised by my friends and I.

The rules states that "If he does not gain any cards, the investigator may discard one card of his choice from the reserve."

The reference guides states "If he is unable or chooses not to gain any cards from the reserve, he may instead discard one card of his choice from the reserve."

May be a foolish question, but does that mean that a player needs at least one success but not able or choose not to buy anything or just making an acquire asset action and can discard a card from the reserve even not making any roll just to re-stock the reserve?

Nope, you can choose to discard even if you roll zero successes
