If a Killteam activates Squad mode abilities that are sustained they proceed to receive the benefits of this ability without paying the costs again during the mission. Does this also apply if they loose Squad Mode in between? Like, for example, they lost all cohesion for some time, regain some, enter Squad mode and get their 3 sustained abilities back? Or do they have to repay the cost if they lost Squad mode?

They retain them, just can't use them temporarily. They only can't use them while disorganized. When they get their act together, they can coordinate again. Also don't forget that one player can always expend one Fate Point dedicated to restoring one point of Cohesion, enabling the sustained powers again.
