how do i get the kiri promo i am unsure i several boxs but no clue on what i need from them to send them in... please describe what it looks like and where it is located please and thanks.

on the back of the booster packs from set 1 and 2 are heart points. they are worth 2 points each. there is a form somewhere to submit them but they have to be in by the 31st
