I have run the demo scenario "A Day Late, A Shilling Short" twice and both times i have had a rough time with the relative positioning. It hurt my brain.
So how do you all handle relative positioning?

I find the explanation in the rules quite good, but perhaps you have some more specific question? Any special aspect of the system you are pondering about?
I use the range markers that come with the reference sheet here at Headless Hollow ... (last page)

Just having these made it much simpler, IMO.
Hmmm maybe I misinterpreted the question. I was under the impression that you were asking in regards to positioning the miniatures, somehow. The Abstract range is pretty easy to handle and with those range markers Gully mentioned it's easy to lay out on the table. Use location cards as well for different areas. I use the location cards as abstract locations as well. A burning building can be anything where those mechanics on the card would make sense.
