Anyone skilled with making record sheets?

We could have one with collumn headings like

Turn Entered From Room Type Encounter/Effect Exited To Foe (W) Damage Taken Loot

Thoughts?

I have a little notebook in the box where I'm tracking the plays as far as who was used and what happened to them (exited/killed/etc) and by what or how much loot did they keep.
