There's a difference between "~: Focus" vs. "Become Focused"

the 1st one grants the condition from a surge ability, so it does not apply if the target suffered no damage,

Kayn uses the 2nd version and grants it directly, so it doesn't matter if the target suffered damage or not

See above. Squad Command is not using the focus condition keyword, it's giving focused .

Condition keywords ( focus , hide , bleed , stun , weaken ) grant the corresponding conditions ( focused , hidden , bleeding , stunned , weakened ) when the attack resolves if the target suffered damage from the attack.

Conditions ( focused , hidden , bleeding , stunned , weakened ) which are granted directly without using condition keywords do not require that the target suffered damage. There are also plenty of abilities that grant conditions outside of attacks.
