All I see in the rulebook is that it stays on a disk until it's removed but other than the disk being destroyed I don't see how else (plaugebearers the exception) that it gets removed.

Does it just stay until an effect or being destroyed happens?

Has that been confirmed by FFG? what is the order that tokens are removed? Aren't empowered and damage tokens removed at the same time? so they couldn't take the wound if the lose of toughness and the loss of damage happened at the same time.

I haven't checked with FFG, but there is nothing in the rule book that I can see about the order in which tokens are removed. It looks like they should all be removed together, so it would not cause a wound.
