C1 + R1?

Can someone clarify how multiple stances are handled?

If C1/R1 means to covert characteristic dice to one or the other, what does C1+R1 mean?

YOu use BOTH dice.

One red and one green. Some of the monsters have this.

I am not sure what you mean here? This does not in any way increase the amount of dice used, just what KIND of dice are used.

Example:

Monster with strength 4 and a C1+R1 listing would use this dice set for a basic melee attack:

Rolled: 2 Characteristic, 1 Conservative, 1 Reckless

This brought up something I'd somehow not considered. Obviously fatigue and recharge applies as normal, but what about success/boon/bane results? Do you pick a side of the card? GM discretion is fine but I'm just making sure I haven't missed some rule somewhere.

Yes. You pick whichever side you think is more advantageous to the monster (if you're like me :)
