[Ressource] 14-Page Personnel File

Hey Folks.

A while ago I created a one-page Personnel File "Sheet" for flavor reasons, and used it to spice up some NPCs in my Only War game. The whole thing fell flat, though, until I recently started doing stuff for Rogue Trader and found the Personnel File again. I decided to pump it up and created a now 14-page strong personnel file anyone can fill out for PCs or NPCs alike.

So far it contains the following pages:

  • Form 3108; Active Duty Soldier Review (Short Overview of Current Status)
  • Form 3109-1; Draft/Enlistment Form
  • Form 3109-2; Personal Details & Dependents
  • Form 3109-3A; Medical Record Page 1 (vaccinations, current status, bionics, last medical check-up)
  • Form 3109-3B; Medical Record Additional Pages (x2), detailing previous wounds and surgeries
  • Form 3109-4; Equipment and Replacement Record, detailing the Soldiers Kit, Special Gear and possible replacements handed out over the course of duty
  • Form 3109-5; Training and Qualifications Record
  • Form 3109-6; Awards and Citations
  • Form 3109-7; Service History (x2)
  • Form 3109-8; Remarks
  • Form 3109-9A; Termination of Service by Death
  • Form 3109-9B; Termination of Service by Discharge

If you were so awesome, lend a hand: Tell me where I made errors of any sort, typos and the like. Tell me what is missing or what you'd want to round it out.

So far, I've gotten a bit of feedback in other places as well. I will create another version including lines to write on for things like the Remarks or Awards and Citations, so it's easier to fill out in handwriting. I will also provide these versions in two variants: One printer-friendly as you can see it, and one including a parchment-like background.

I am also considering to offer the Source File (*.AI format, Adobe Illustrator) once I figured out the compatibility with prior versions.

The newest version in all variations:

Files are up on the server, as promised. Including a small fix, bumping version to 1.2, and lowering filesize across the board, especially for Version A and B.

Thanks for the praise, folks. Anyone have any criticism or suggestions so far?

I'll try to finally get around to get the source material up and running soon. I might also try to see if I can make a fillable version, although that might take a fair bit of time. Never done it before :|

Outside of that... any criticism mentioned I will be happy to consider carefully!

Edit: I will also provide a fully filled-out version as example, showcasing a character from my Rogue Trader game that is starting soon.

These are great, I am actually having the players in my game fill some of these out. Also when a character dies I fill out the Termination of service by death sheet and give them a copy.

As a question to those more in-the-know and long-timers on the boards: Would it be considered bad form to post this in the RT and DH forums as well? Both games feature plenty opportunities for characters that did their duty in Imperial Service, but at the same time I do not want to spam.

In short: Cross-Forum Posting is ok or a no-go around here?

Very cool. Maps and documents really do help make things immersive if used right.

The only typos I can see are on 3109-3B - which uses "cursade" not "crusade".

I may be making use of this myself, soon. The campaign we're starting (Dark Heresy) involves some records of naval personnel which have gone astray (along with - as the players will later discover - the personnel themselves).

Making up a couple of 3109 jackets - complete with 3109-9As all of which seem to have been filled out on exactly the same date by the same CO (who won't be the same CO signing off the rest of the file) will be the sort of things the players can spot themselves.

As for the usage: I'm thrilled! Having them used as actual props in-game is awesome :)

If you want and can, make a snapshot of the files and post them? I'd be very happy to see that :D

Update later today incoming.

Changes so far are:

  • Fixing a header
  • Typo fix (Crusade was misspelled)
  • Adding 4 Variations

Variations are:

  • Var A: No Background, no writing lines
  • Var B: No Background, additional writing lines
  • Var C: Background, no writing lines
  • Var D: Background, additional writing lines

That should make it easier for people to pick exactly what they need. If you want to murder your printer and fill out things via handwriting, pick D. If you want to spare your printer and need no pesky lines, pick variation A!

I will upload them once I get home and settled.

Quick info update!

Apparently my mail-provider didn't like the forum and didn't show me a couple conversations people started about wanting access to these files. I am deeply sorry to have responded so late.

Anyways, I finally have a new server up, and once I get home from work, I will be uploading all relevant files and put the direct links here and in the other thread.

I plan to do a very basic website for all of my RPG materials, including Exalted, D&D and others (possibly including handmade maps of mine) within the next couple weeks, as time - and lacking skills with HTML/CSS - permits.

Expect a posting to the direct-links later today.

Is there any way to get an editable version of this? If not do you mind if me or a friend alters the file ourselves to make it as such?

currently a volounteer with some experience is trying to create a fillable version of the document. You can also try your hand at it, and if you can get it done, I'd host it as well, including the proper credit obviously.
