It looks like a modified version of what mrvander posted a while back. It could be his 'ver 3.0'.

It's based on mine, but not mine. I'm not sure if there was a thread where it was officially shared with the forums, but user Callidon created that and asked me privately about sharing it (which I have no issues with since the more we share as a community, the better.)
