do I lose sanity even if I fail the spell check? (sorry for the n00bish question haven't played for awhile)

Yes. You pay the cost before you attempt the spell. So it is gone whether you pass or not.
Also, if you go insane and pass the spell check, the spell still takes effect. Sometimes, this doesn't do any good (like combat spells).
I've always played it that you kill the monster in a final blow, then the white coats find you playing with its organs.
this is why we never play spellcasters (unless its daisy) too much pain for too little gain
Starburn, the official word is that in Combat, if you go Insane paying the cost for a Spell, the fight ends and you go to the Asylum or Lost In Time And Space if in Other World.

The spell has been summoned and is taking its course. Doesn't matter if you're insane now. If the spell happens to have an effect that you need to be conscious for, then it'll do nothing.
If casting Denying the Ancient One drives you insane, it still takes effect.
If casting Shrivelling drives you insane, it still takes effect. But combat ends because you're insane so you never get to use its effect.
To answer your original question, I think I remember Kevin saying something about it on the old-old forums.
That sounds a bit too much like "theme" over "gameplay" to me. Not that I don't do that myself, but I have to question it when it's on such a large scale. "The spell has been summoned and is taking its course." No, it hasn't. You haven't rolled your Spell Check yet, and you're officially insane. You can't get on the Check train unless you've bought your Sanity ticket, and you'll never make it to the platform.
If Kevin said so, then I'll wait for the FAQ to black-n-white it.
He did say it, and said it was going to get put into the next FAQ, but then disappeared from the old boards, and the FAQ was never updated.
It follows the same concept as the Elder Sign, where it would be useless to even use an Elder Sign because it would be discarded with no effect if you go Insane or Unconscious.
You want theme?
A combat spell doesn't just require casting it for it to work, it requires you to direct its effect after you have finished summoning its power, hence the hand requirements. By going insane, you lose your mental focus, and the spell fades away because you aren't focusing on it anymore.
While a spell that summons a concealing mist to hide you from a monster still hides your incoherent babbling-self long enough for the monster to lose interest, and some random good samaretan drags you to the Asylum for treatment, because you incedently saved them from the monster as well with the mist.

Yes, effects once started are usually not interuptted.
Not sure why the thread isn't stickied. I asked before but nothing came of it. Guess I never asked again. I'd like to start a new thread so I can edit the first post. I don't think I can do that on the old threads in this forum. I'll look into it.
It would probably be easier to start a new thread with a slightly different name, and just copy and paste it.
