Hello everyone

Has anyone else had a problem when submitting their servitor aopplication form using the following link?

I am getting frustrated at completing the form and then getting an error message when I click too submit. (I am an old servitor trying to sign up with a new store). I have tried this both at work and at work with no success.

Help anyone (moderators?)!
