It is for adults because at one part you are helping the sigmar church to torture a cult member and also have some decision to make which will lead to a lot of dead people if you fail.

Also in the end you have 3 ways to enter the undercity which most of them are mentioned in the Path of the damned adventure for 2nd Edition. So it feels quite right if you play this adventure.
