The Weapon Frost Axe is always on & says Melee: Your hero inflicts +1 wound in melee combat.
Does this mean your Character must attack in body & win to inflict normal melee damage then +1 for the Frost Axe.

I would say yes.
You have to hit the Enemy to inflict any damage, with this and any other Item.

Yes. You must have a sucessful body(melee) attack. Then you do your damage +1.
