In the back of the game manual (rule of play), there is this blue badge that displays "FANTASY FLIGHT SUPPLY [5] PROTECTIVE CARD SLEEVES" ( [4] for expansion). What does it mean? Is it a gift card for protective card sleeves? If it is where should I send them, in order to claim the prizes?

No. It is just an indication on how many packs of FFG sleeves and what size is needed to sleeve all the cards. In this case, 5 packs of sleeves - blue size.
