The MIU is one of the most sacred and hallowed pieces of technology the Imperium possess. It allows you to interface directly with a machine; think what that means to the Ad Mech.
No simple citizen is going to have -or be able to afford- an MIU.
I'd consider it to be near-unavailable for common citizens and reachable for those backed by a noble or merchant house. It'll also see some use among the more high-tech guard regiments.

Although there are examples of DNI (direct neural interface) that don't really have the attributed sacredness of MIU...

Indeed. Betancore AND his daughter had MIU implants. However pilots are not realy ordinary citizens in the Imperium, they are usually connected to the Imperial Navy in some way, or a Guild or House of some sort. Although pilots are not masters themselves but a very fancy version of a servant (driver for the wealthy), they are not as common as the trudging and poor masses.
Pilots are the closest thing to a regular citizen that you're going to see regularly sporting an MIU. Even then, I'd bet in many cases the pilots don't "own" their implants. Quitting is rather hard when you have to turn your head in.
Are you guys sure about MIU's and Glavian pilots? I'm almost 100% sure they were "just" electro-grafts.
There are different levels of MIU just like their are different levels of Bionics and Cybernetics.The lowest level and the most commonly available (which are still incredibly rare) are those that are tasked to interface with certain types of machinery in certain ways such as the Glavian wetware which allows for linkage through the hands essentially bonding a persons reflexes directly into a machine be it a gun, plane or spacecraft. These pieces of Tech are "owned" by the house or guild and are gifted to a small percentage of their most promising members. This type of device is only a MIU in a very basic way as the link is mostly subconscious and is cut down on reaction time.
At higher levels of sophistication MIU allow for a wider variety of options such as interfacing with any type of Imperium cogitation device assuming the user has the proper training and the device has a suitable link.
Then their is the peak of MIU technology which is usually reserved for the use of Adepts within the Titan Legions who become so intertwined with the machine spirits they link with they begin to adopt traits associated with thier titan and from the residual thoughts of those who have lived as a part of the machine previously.
Are these things available? No, not really its all hard to find tech that the Mechanicum occasionally licenses a planet/guild/household the ability to use or manufacture and then its the simpler devises. MIU are a way for a Mechanicum Adept to "Commune" with things imbued by their god and thus are holy artefacts given to those outside their control only reluctantly or when it serves to further the goals of the Machine-God.

The Inquisitor's HAndbook has the Vehicle Interface Circuitry on p. 140 and lists Galvians as the most famous example for this kind of implant. So it is different from MIUs, a little more expensive and as rare as those.
I'm curious, what is MIU? The closest I can think of given the previous comments would be Machine Interface Unit. How close is that?

Ah, you, good sir, are correct! I did not have my books with me but it shames me none the less.
However, I think my version sounds better so I'll keep using that one.
