Question about Things in the Ground + insanity

Some questions came up with our first games. I know Things in the Ground will put characters in play insane even with willpower/terror icon (not sure of the reason tho). What about enters play effects, do they trigger with Things in the Ground (example Hungry Dark Young) or will they not trigger because text boxes are blank when they enter play? And is there difference if enter play effect is passive, response or forced response.

Different question is that rulebook says cards with terror/willpower cannot go insane by card effects. I guess this is absolute. Is there anyway terror/willpower characters being insane (not counting removal of terror/willpower)? Like how this works with Stygian Eye? What about Apeirophobia targetting terror/willpower character, my opponent has to choose the discard option?

Also I see focusing to insanity and terror with a deck very weak because it only works against specific characters. I don't see any reason to pick insanity deck over a Cthulhu destroy deck.
