The way it is written, I would assume 0d as well, but how would they generate the negative side effect of 2 banes? Reckless stance I suppose...
Anyone got a more official answer?
That's what I thought as well, until I read about the bane effect....
The rules for permanent insanity recovery are very inclear. I think they simply forgot to play test them or proof read that section.
I have the PC pay for a month in an asylum (prices in my house rulebook), and have them make a check then.

for temporary the check difficulty is based on the number of tokens your have on the card, you get one each time you suffer one more stress than twice your WP i think.
for the permanent we also havent found a way yet. first we thought the difficulty was based on the severity, but how should a character with WP 2 overcome a severity 4 insanity then?
we now base the difficulty on the conditions under which the check is made. is it at a shallya temple or hospital like place its 0d, in a gasthaus for example it might be 1d, increasing to the maximum of 3d i think. always depending on how "well" you feel in the place you make the check
