Just wondering, the plot "take them by surprise' reads "if you lose initiative discard your hand'. Does this mean with 4 players just the person with the lowest initiative loses, or does everybody but the highest lose their hands?

In this case "loses" equals "doesn't win". Every player with Take Them By Surprise as their revealed plot card who doesn't win INI will discard their hand. If 4 player all flip TtbS, three of them will discard their hand.

Do note that the "you" mentioned on the plot card is "the player who revealed this plot" -- if it meant every player, it would say "every player" or "each player" or "all players".
