If Catelyn Stark (LoW) comes into play by her ability then is killed and then resurrected with Narrow Escape should she return to its owner's hand or stay in play?

She should stay in play. Cards that have left play have no "memory" of anything that might have happened during play, like what attachments they had, which icons they lost, and, in this case, how they got there in the first place.
