If I have an effect that raises the claim value on my revealed plot card (e.g. Pike Phalanx: Limited Response: After you declare Pike Phalanx as an attacker with at least one other character with a crest, raise the claim on your revealed plot card by 1 until the end of the phase.) and afterwards I reveal a new plot card, would I lose the raised claim value? I assume that this is the case, because the lasting effect -- unlike claim-raise effects that are tied to a specific challenge type, for example -- seems to act on the entity of the plot card and would not transfer over to a newly revealed plot.

While i assume that i will shortly be corrected i would assume that it DOES still apply, the effect from Pike Phalanx lasts till the end of the phase, most likely the challenge, so it is active till its end, if your plot is changed then the newly revealed plot card is your revealed plot card. and the effect still applies.

That is a tricky one. Does "revealed plot card" refer to the particular card that is currently revealed or to whichever of your plot cards is currently in the "revealed" state (which is an unambiguous distinction since you never have more than one such plot card)? I could see that going either way.
"Challenges: Characters you control get +1 STR until the end of the phase." After you trigger that, you drop a new character into play. Does it get the +1 STR? Sure, because the lasting effect is still there.
So since the Phalanx is also a general effect - rather than a targeted one - it will last for the effect's duration, regardless of whether the identity of "revealed plot" changes or not.
If it said "currently revealed plot," then I would buy that the effect is specific to the current plot rather than to the "revealed plot" state of the plots you control.

Viper's Rage is not a targeted effect either.
I am not sure that affected characters are variable, but if it was the case this FAQ passage would apply:
Constant Abilities will constantly check and
(if necessary) update the definition or count of
their variables.
Triggered Effects and Passive Abilities define
or count their variables once (when the effect
is initiated), and the variable is then constant
throughout the duration of the effect.
Lasting effect would thus apply on cards in play when they are resolved and not cards coming in afterwards.
