so I have a question about the mutation chart, lets say I roll "ravaged body" and get d5 mutations, I roll a 10 so I get 5 mutations, on the fifth one I roll "ravaged body" again, now it doesn't say to re-roll if I get the same result twice, so I'm just wondering if I would get another d5 mutations or just re-roll that last one.

you would re roll until you get a non duplicate item or one that doesn't contradict the effects listed in the mutation description of a previous roll.

From the rulebook: Whenever a mutation occurs, roll on Table 14–3: Mutations to discover what type of mutation is gained. Always re-roll duplicate results.
