what happen to a resurrected monster when

a) the hero who control it(necromancer) go to town [does the monster crumble?]

b) the hero controlling it die [does the monster crumble?]

No to both. The monster stays alive as long as it rolls enhancements after its activation.

Even in an AC outdoor encounter it will presumably activate after the controlling hero died (there is a ruling that familiars still activate in such a circumstance).

Agreed. There's no condition on the card about the hero remaining alive/in the dungeon to maintain control of the monster. The player is still in the game even if his hero is temporarily out.
