Sloppy craftsmanship, most likely caused by a too ambitious release schedule. It's a known issue. You can use it create some over the top plot of your own choosing if you like - like there actually being two ravacene, the one being a mirror of the other. The warp is an enabler of all kinds of craziness. Or consider one of them non-existent.
Or consider it an error in the maps of the Deathwatch. It happens.

Or treat them as two different planets which have both been named Ravacene by the locals. If they're close enough, maybe they even have some sort of rivalry about it. "No, WE chose the name first!"
Anybody familiar with Canadian football would probably get a kick out of a Rough Riders reference or two. =P
It's an Administratum recording error.
The map has quite a few issues.
I went with Hestus being closer to the Warp Gate since that is where its shown on the inside-cover map.
