That's a good question actually.
I would guess that juvenat treatments might be part of a later supplement or Rogue Trader (since RT seems like it will be geared towards more important, higher level characters who would have access to things like juvenat treatments).
I don't think juvenat treatment costs have ever been mentioned anywhere . It's just, sort of, assumed that every high ranking person has access to juvenat drugs.
I'm not aware of any hard information on rejuvenation technology in the Imperium. In my own campaign the wealthy and important people who can afford of require rejuvenation go to a special kind of medical spa where they are checked out and eventually spend a prolonged period in a cellular regeneration tank that turns back their biological age back a few years. This is followed with cosmetic surgery to remove the physical signs of age that the cellular rejuvenation process cannot counteract, such as bags under the eyes and the elongation of the nose. As a result, most of the 100+ year old characters in my games look like they have had a few facelifts too many.
Of course, on some planets there may be a drug that one takes every night before sleep that halts the aging process. One others the long lived may go in for elaborate surgeries where old tissue and organs are removed and replaced with transplants or vat-grown replacements.

Well, at least it should include the chance to receive some insanity points to reflect the possibility that the rejuvenat patient will slowly go insane over the course of a few hundred years of existence.

I do like some of the stuff he came up with. Thought I do think it's kinda limited as well.
I also think his take on average lifespans is a bit low and limited.
IMHO I would personally think that the average joe blow Imperial born human who grows up with average to good medical care, diet, decent exercise, average healthy living, and very little outside influence to have an average lifespan of 85-90 years with natural maximums at 125, and extremes to 130.
As for hive world, and void born, I don't think you can put a generic average on either due to a great number of factors that come into these people's lives. In fact it's quite possible that a "natural" (used very loosely) lifespan for both may be as little as 25 years, to as long as 200. Other than the general well being of an individual, you need to take into account everything from genetically enhanced food, advanced medicine, to the flow of time (especially for void born).
