In the rulebooks, when a monster is given and has a melee weapons that lists damage as something like 1d10+4, and then the creature has a SB of 3, do you add 3 more to the damage? Or is that 3 extra damage already included within the profile?

Generally, it should be applied already. Some talents that are context specific are not applied.
