Does the monster with Undying try to resurrect more than once or is it only allowed to try after its initial death?

Every time it would be killed, you make the roll.

In vanilla, if you do enough damage to it, it might even make multiple Undying rolls in response to a single attack. (In the extended campaign, Undying is subtly changed so that extra damage doesn't carry over.)
