When there are 3 Dunwich Horror tokens on the board the Dunwich Horror enters play. What happens to the tokens, are they removed right away or do they stay on the board until the Dunwich Horror is beaten.

the tokens stay there until the horror is defeated. after that all of them will be removed and the happy waiting/filling for the horror starts over again.

I always assumed that they stay out until the DH is beaten. That way, if some weird effect removes the Horror (such as corralling it into a vortex using unorthodox monster-moving effects), it will instantly re-appear at Sentinel Hill since the three DH tokens are still on the track.

When the Dunwich Horror is in play are Monsters that go into a Vortex just returned to the cup and nothing else happens.

that would be the consequence, yes. :-)
