In the errata, it says that if I have Lightning Attack and am dual wielding then with one melee weapon I am allowed to make three attacks, but only one attack with the other. So let's say I put an extra grip on a Combat Shotgun, allowing me to wield it in one hand without penalty. Would I be able to make a Semi-Auto burst with the Combat Shotgun, as well as make three melee attacks with the sword in my other hand all in one round?

No, it was errata'd so that many of the talents only count if they are specifically using a pistol - Swift/Lightning Strike are among them, so you may not.

This would not be allowable, as the extra grip does not turn the weapon into a pistol, and only pistols can be fired in melee (therefore the example with a melee weapon is right out).
On the other example you asked with a auto pistol, there was a recent ruling in Deathwatch stating that the use of anything but single shot is not possible in melee combat. It has been stated that this now applies to all systems. Of course, its up to your GM (or you if that is you), but it in a way makes sense (both balance and simulation wise, given the notion of the swirling melee).
Now technically, I think you should be able to dual wield 2 shotguns with extra grips, at range, at the regular penalties.

By RAW, yes, by recent ruling, no. Errata states they may make a "single attack", but it doesn't note what type of attack it means - ie: standard attack or full auto burst. I treat it as a standard attack, but its GM fiat.

We'll, FFG bigwig Ross Watson responded to direct question on the matter and clarified that pistols in melee can only be used for single shot RoF (regardless of offhand or not).
Plus the various versions of the Errata clarrified in other instances that a 'standard attack' refers to a single half action attack in entries like the Ballistic Mechandrite.
Obviously you can do what you want in your games.

I know a lot of people that won't take it as an actual ruling until its in the errata - they know who he is, but they also take issue with many of the forum clarified responses. *Shrug* Hence why I said as it currently stands in RAW, it simply says a single attack; an official response changes the RAI as you now actually know what they meant to say.
Ballistic Mechandendrite never actually said a single attack, just said it may be treated as an off hand held weapon - if its just being treated as an off hand weapon you could use it with things like Lightning Attack, so they clarified that you must spend a Reaction and only may make a Half Action attack (So that it may only be fired once per round). If there was any instance of them clarifying a Single Attack than it would largely end the argument, and I cant see a clarification on that anywhere within errata.
