I could not find this in the rulebook. What happens to Incident and Location cards after they are used? Are they shuffled back in to their respective decks immediately, placed in a discard pile and shuffled back in only when the respective deck runs out (I have not played RTL yet, so I am not sure even *if* these decks run out), or removed for the rest of the campaign?

There are no instructions, so nobody knows.
I think most people probably remove them, the same way they remove used dungeon levels,
purely in the interests of variety
. We (the group I play with) leave in location cards, but remove incident cards and dungeon levels. But the rules are totally silent on this issue.
We do the same thin. There are enough incidents to let you send to the graveyard those that are already done, but there aren't enough locations, I believe, to support a whole campaign, so we reshuffle them. Besides, it'd be strange to fight again and again the same enemies, don't you think?
Do you think that this is a good question to the FAQ? This is not specified in the rules, so it would need a clarification.

For me, this one is in the 'who cares' basket. It isn't like it matters a great deal how you choose to deal with it and it isn't like anything hangs on when you make the decision either.
Whoa we never even considered that but there is nothing to imply you can't draw the same incident twice. We only do that in a dungeon where we keep the previous level outside so we don't play the same dungeon level twice in a dungeon. But on every other aspect we reshuffle. Not reshuffling incidents seems weird since THERE are limited green, red, yellow incident cards and i can already see the players using that to get more easy the mad duncan encounter and i would not like that either as a player or as a OL. After some time you just know what you are going to get in a incident after cards have been thrown out. Really weird that i see 2 players using that home rule as it would never cross my mind to do that. The same can be said if you do that with dungeon levels if you throw them out once you play them you just know what kind of dungeons to expect. Anyway it is an interesting rule and i have learned to keep my mind open on any suggestions and we might try that on our next campaign as far as dungeon levels are concerned in order to finally play all the dungeon levels.
Considering we mix Dungeon Levels from RtL, ToI and SoB, I beliveve there are something like 70-80 dungeon levels disposible, meaning that if someone knows what will be next, either he looked at the deck or he's a frigging esper. Encounters are less, of course, still we can't see how can you finish all of them or even come close to finishing them...on the other end I believe that playing multiple times the same encounter/level against the same monsters/rules would be definitely boring. Again, both Dungeon Levels and Encounters include named monsters which would be absurd to meet more than once...this is especially true for Dungeon levels...it's like:
"Hey Talhia! Weren't we in this same cript with the exact same guy who had those relationing problems like...twenty miles from here?"
Makes no sense...I belive this SHOULD be FAQed, actually.
