Can you waltz into a town and train a skill while it is under siege? I apparently don't have a rulebook. Or... can you cut through one to visit a secret area to upgrade skills and fatigue?

Yes. All buildings are still open until a town is razed. That includes training grounds. The rulebooks are all available free online here .
