so your san/strength goes down to zero and you're off to the hospital or asylum place... or lost in time and space and it says you loose half your items. Is this referring purely to common and unique items, or does it include spells and skills too....?

Think of spells as scrolls or parchments you keep referring to when attempting to cast a spell. Hence, they are items.
