With the Dunwich Horror addendum another mask is added to the mix. So when Nylarlathep is the ancient one do you use all 6 masks or randomly pick 5?

Use all Masks. A new Mask monster was included so that the chances of drawing a Mask from the cup weren't significantly reduced when you added all the Dunwich monsters.
When you get all three large expansions you'll have 10 total Masks.

You use all masks. Unless you don't want to :')
Even with 10 Mask monsters in the mix now, I think my group has only ever drawn one or two ever. I'm never afraid of a game against Nylarthotep.

It may be true statistically that Nyarlathotep does not often get to let his masks out to play, but anyone whose ever been surprised in an Otherworld by his Bloody Tongue when it wasnt even in the cup knows the terrible truth...and there have been quite a few times when not only that has happened to me (at least 3) but also a few where I had 3 or more masks turn up one after the other to stick it to me just when it looked like the team was in the home straight. Plus, I have been creeped out countless times by the thematic appearance of masks, such as the Dark Pharoah turning up with the Egyptian museum exhibit in M.U. - or some other unlikely nonsense. Nyar is cool. And statistics are a plot of the Great Old Ones.
We houseruled that a mask appears each time a monster surge occurs. Just make a separate monster cup for all the masks.
My group always sprinkles the ten Masks over the top of the other monsters in the monster cup; we generally get 3-6 Masks showing up in a game that way.

You can also remove monsters from the cup at random before the game begins. This increases the chance of drawing a Mask randomly from the cup. Just don't look at the monsters that you remove. That wouldn't be any fun. You might want to make sure you leave cultists in, since his cultists have special powers.
You could also make more Mask monsters. There were some pretty cool ideas on the old forums:

I've printed about seven extra Mask monsters. Before a game against Nyarlathotep, I blindly remove 15 (non cultist) monsters from the cup and return them to the box. Nyarlathotep is now one of my favorite AOs and he's pretty challenging.

it should be an official update for Nyarla... because is very often that no mask is drawn from the monster's bag in a regular game. :/
I've also made a herald for Nyarly that addresses this, Dweller in the Dark. Using this herald there is always a Mask on the board.

I disagree. Such a rule would prevent Masks from randomly appearing at encounters, and most of them are threatening only when they are not anticipated (God of the Bloody Tongue when your Sneak is low, The Black Man when your Luck is low, The Bloated Woman when your Will is low, The Dark Pharaoh when your Lore is low). Having them appear only through monster surges would actually make them very easy to evade / defeat when necessary.

Maybe a good compromise would be to divide your masks in half, and only put half into the bag. Ones in the bag come out when randomly drawn, and ones that aren't in the bag are Spawn monsters that appear during a monster surge.
Maybe for additional fun, whenever a Mask leaves play (defeated or otherwise removed), it switches which of the two types it is. A defeated spawn mask will return to the cup, and an evaded/defeated/banished regular Mask will go to Nyar's sheet as a spawn monster to be used later. And of course, if there are no available spawn monsters when one is required, add a doom token to Nyar's sheet.
