If Nyarlothep is the AO, and the God of the Bloody Tongue is already on a site in Arkham, Dunwich, etc. do we remove him from that location for this encounter? If so do we return him to that Arkham location after the combat? Also, if he is defeated and he is a "trophy" do we take him from the investigator who has him as a trophy? If so, do we return him to that investigator if the one in the other world also defeats him?

You take him from where ever GotBT is on the board, use it for the encounter and return it to where you took it. You can't trophy Masks.
Masks have the Endless ability. They cannot be collected as trophies.In rare cases the Endless ability has no effect.
Examples:you kill him with a Petrifying solution,( a one-shot weapon that ignores Endless)
have an Ally that lets you ignore the mask's Endless ability.
if an encounter allows you to take the monster as a trophy ignoring its Endless ability.
