Do you generate fatigue on initiative roll?

It all depends on if your GM spends boons/banes on initiative. If you can recover stress/fatigue from boons, then you should be able to gain it from banes.

Unless, of course, you are talking about getting an exertion result from Reckless dice. I would say yes to that, unless the GM is ignoring delay results also..

Yes. Two banes, or a blood on a reckless die, will give a fatigue on initiative. Of course, it is unlikely that an initiative roll will include one, let alone 2 misfortune/difficulty dice, or a reckless die ... although it could happen.

It should also be noted that 2 banes doesn't automatically mean taking stress/fatigue. It's up to the GM what banes are spent on.

While this is certainly true, initiative has no bane effect lines (unlike action cards). So, while the GM certainly has the perogative to ad-lib a bane effect, following the rules I believe that is technically the only bane effect usable. This is excluding things like criticals or Conditions that give a player a general bane effect line for physical activities of course.
