Does it not work Stark v Stark?

It doesn't work against Stark characters. So you'd be able to kill neutral or out of House characters your Stark opponent is running. Generally speaking though, if your facing a Stark player and have Bear Island, most, if not all, of your opponents characters will not be legal targets as they will probably have the House Stark Shield on them.

This is one of the weakness of Bear of Island.
