The only time a phenomena is triggered is when you are rolling the d10s to meet the threshold to manifest the power.

Psyniscience tests don't cause phenomena because they aren't tests to manifest powers.

Secondary rolls (damage, willpower tests, etc) don't count because they come after the power has been manifested.
