Use the round system. A round ends when all payers overlord included have played. During à round when à hero perform an attack outside of its turn, verify the conditions. If the hero was adjacent then add 1 heart otherwise no.

Simple.

I'm not sure what you mean by "development way". Are you talking about his heroic feat?
