Question about this power, can this be used as a reactionary type thing to when someone rolls to hit you/someone else and you are afraid they will die?

Base power, no.

But the bottom Control upgrade allows a D-point to be spent to use it as an OOT Incidental.

correct, the bottom left upgrade, Control incidental/maneuver. The real issue with it is when it is done, is it done when the person is targeted with an attack or can you wait for the roll kinda like "shield" in DND.

Out of turn incidental is done when the player decides to. Which most likely will be when the attacker successfully attacks. As there is no point if the attacker misses
