Simple question. When multiple runners are cooperating, a Lockout affects them all. Does the same go for code gates? If not, it seems like the runners still in the system could allow the kicked user right back in, but letting one piece of ice boot the whole team at once also seems kind of unfair.

If booted, the runner would have to re-enter the system. Since they're no longer in the system, I don't think they can be counted as "part of the encounter group". Once they re-enter the system, though, they're "grouped up" again.
"If you are running a Network encounter with multiple runners and sysops, use the following rules.
• When accessing the system, only one runner needs to successfully perform the access system check."
So clearly one runner with access can piggyback others into the system. If at least one runner is still in, I see no reason to make the kicked runner make an access check again.

Yes, they successfully performed the access system check and got in. Then they got booted. They're not in anymore and thus need to reenter.
And the reason that I see is exactly what this thread is about: code gates loose their One Unique Thing if you don't make them access the system again.
Again, this is all my opinion, since there are not set rules for this.

It says nothing about the action, implying that each character must take the Access System action, even if they no longer have to make the check. This does slow down the speed at which they can break ICE, but also keeps them more on-par with a solo Runner.
In that vein, I'd say that even if part of the group is still online, the booted Runner could piggyback inside again, but would still require taking the Access System action. This lets Code Gates keep their "One Unique Thing:tm:", while also still giving them the base advantage of running as a group.
Just an alternate interpretation of the text, possibly food for thought.
