So that would mean I would recover 2 health and pick 1 of the other 2 options to apply until the end of the round?

You can't resolve another ability until you have resolved this, so yes, it seems the choice between surge and evade should be performed when resolving Smuggled Supplies. (The ability would be worded differently if the intent was to choose for each attack the figure performs or is defending for.)
