How do you "Fail an action"
Depends on the action. Only the ones that can fail are listed.
Boost: the end position in the chosen direction is blocked by a ship or obstacle, and/or the template crosses an obstacle. Some upgrades and ship abilities can make it possible to boost even if blocked by an obstacle, and one ship ability makes it possible to boost even if blocked by a ship.
Barrel roll: the template crosses an obstacle, and/or all 3 possible end positions are blocked by an obstacle or ship. Same applies re. upgrades and ship abilities.
Co-ordinate: no friendly ship is in range. If a friendly ship is chosen, the action succeeds, regardless of whether that ship chooses to do an action, or if the action it chooses fails.
Jam: no enemy ship is in range.
Lock: no objects *at all* are in range. It's almost impossible to fail a Lock action.
I think that's all the default icon-actions that can fail currently.

Just to add: most players don't put a token down even if they would have to, and in most cases it doesn't matter. But there are several where it does (Composure, free red actions, jam, ...)

Special weapons with the ordnance symbol do not. All others do.

S LAM action fails if the final position of the ship would cause it to flee.
On p11 of the Rules Reference (1.0.2) SLAM is also called out in the list of actions that can fail. You can find the above definition by turning to that entry on p17.
