Roland Banks made it to the 3rd scenario of the core campaign, got to the cave, and went insane. What happens now? Do I start that scenario again with another character or start the whole campaign again?

It’s kind of up to you, really. Normally, if your character dies you can continue on to the next scenario with a new character, unless the campaign explicitly says otherwise. But, as you died in the final scenario, there isn’t a subsequent scenario for someone else to step in, technically that’s the end of the campaign and you should start again. But it’s your game, playing solo, so if you wanted to have someone pick up Roland’s notes and retry the final scenario nobody is going to give you a hard time about it.
Personally, I’d take the challenge of restarting the whole thing, either with a tweaked Roland deck (if you have any spare cards to do so with), or someone else. But if you don’t want to play through the Gathering again for a while, that’s fine.
The other option is to just play the third scenario as a stand alone scenario. Entirely up to you! But whatever happens, any experience Roland has earned already is lost and can’t be transferred.

Thanks, that makes sense. I might go to Carcosa next, as I have that. I can decide after about Night of the Zealot.
I will say that your experience is fairly typical. The first campaign has a very steep difficulty ramp. Night of the Zealot is fairly easy, great for teaching the rules and getting a good victory with lots of xp. Second scenario is tougher, more choices, and largely a lesson in knowing when to stop with what you've got. Then the third one is there to kill everyone off. Winning it is tough.
The other campaigns do not follow that model. I really like Carcosa, it's my favorite campaign so far. Several truly great adventures, none that seemed terrible or unfair - though Unspeakable Oath is a bit on the difficult side, but so fun anyway. So, yeah, go for it, and enjoy.
