I am able to have the same crew member as the pilot. Example, Kestu Onyo piloting the lancer, with a crew member Ketsu Onyo.

Having cruised through the rules I didn't see that limit so maybe that was just 1st edition and I assumed it was the same in 2.0?

Check under Limited in the Rules Reference. Ketsu Onyo has only one dot, so a squad should only legally be able to carry one card with that name.
