I was looking through the sorce books and found a few different shields and sword that can each be one handed. Can a character hold the shield for purely the defensive benefit and use the sword for attack with out increasing the difficulty?

Yes.

Or they could attack with the shield instead of the sword, or make a two weapon attack and use both offensively (check the rules for two weapon combat).
