To be fair, I actually haven't ever died with her yet. But I've only played the carcosa scenarios on easy with her so far.

Upper limit is 5, since both Cheat Death and The Painted World remove themselves from the game.

yes, but wouldn't it be fun to jump in front of bullets and see if you could just to let your friends get through? And although you have 5 potentially, would you be able to hit all 5 and still win? :B

I prefer to be the one firing the bullets, but to each their own.
