Is chaos/warpstone exposure progressive or are the mutations random? Anybody know of a reference resource? Does ToC cover it? I'm going to add it to my next scenario (involving a Nurgle plague ship).

Generally speaking exposure to chaos energy gradually mutates your body and soul over time. I have not heard of anything that references whether or not the effects of chaos fade over time (assuming you have not developed a mutation yet since once you've got one, you've got it for good). But i'd imagine that the longer you have been exposed to chaos, the greater the chances of mutation.
Regarding how people mutate which is what you are actually asking i think is mostly random, such is the nature of chaos. It would seem logical though that minor exposure has a lesser chance of crazy mutations and the opposite would hold true as well.
There are a LOT of tables available for mutations in the 2nd edition books. I also believe that they offer escalating mutation tables where you initially roll on a lesser mutation table and if you roll very high, you gain a medium mutation instead, and again if you roll high you gain a greater mutation (or even a chance of becoming a chaos spawn!). I think this info was covered in the DM book (the basic mutation rules) and possibly the Chaos book (the detailed tables for mutations).
I'll try to look it up for you when i get home.

Some mutations are internal or simply aren't visible to the naked eye. I remember some GW product way back when that had a mutation which was simply 'normal appearance' or somesuch to give the more 'affected' Chaos worshippers out there an aura of normalcy.
Not sure what's scarier, the bandit mutant with tentacled arms or the mutant with regeneration, an extra knuckle on each toe, and holds office in the Church of Sigmar...

I found the tables in a reference sheet in the 2nd edition Game Masters Packet.
There are indeed 3 tiers of mutations ranging from basic to crazy town.

The 2nd edition book, Tome of Corruption has your advanced (d1000) mutation table in it

Also of note, pages 42-43 of Realm of Chaos: Slaves to Darkness has a detailed flow chart of the progression of Chaos Champions and how their mutations can multiply and advance (ultimately transforming them into either a Chaos Spawn or Daemon Prince). It also details specific mutations granted as "rewards" by the particular Chaos God the Champion worships.

Yeah, that's a great one. There's also a story in one of those two august tomes that runs at the beginning of each chapter (like a one-pager) showing how two brohters(?) progress down two different paths in Chaos. One rises to a warrior (or prince) and the other devolves into a spawn based on their choices/gifts bestowed. I think it even has an illustration per chapter on how the two have changed during the progression (believe it's them trying to ascend some corrupted column/pillar?). Been about 10 years since I've looked in either book, so the details are a little fuzzy.
