I'm new to the game and have a quick question about Agnes' ability and clarification.

1. I know the wording of text sounds common sense, but can her ability to deal damage to a enemy be activated if she already has a horror token on her previously?
2. For the 'Limit one per Phase' rule, can she deal 1 damage in each of the Phases, e.g. 1 damage in Mythos Phase, 1 damage in Enemy Phase, 1 damage in Upkeep Phase (I doubt that one would work) etc?

1) No, you only get to deal the damage if a horror is placed on her not when she had a horror.

2) Yes, she can potentially deal 4 damage in a turn at a cost for 4 horror. Note that it had to be horror that is placed on her and not on her assets.

Although technically "after" could literally mean anytime, even 3 turns, after, but in FFG rules it's only immediately after.

You can trigger Forbidden Knowledge during the Upkeep's player window to get an extra hit in.

Painkillers, Ward of Protection (or straight up encounter cards), and enemy attacks are other common sources of horror she can leverage.
