If there are uncancelled hits or crits after the Compare Results step, the ship is "hit" and receives an ion token. Shields won't stop this effect. If the weapon says to suffer damage, shields will suffer first, then hull just as normal.
Once a small ship has at least 1 ion token, it follows the rules on the reference card. It takes 2 ion tokens for a large ship.

It is even possible to get Ion tokens without suffering ANY Damage. For example Ion Torpedoes throw Ion tokens around to all ships near the target should the attack "hit" the target. In short getting an Ion token doesn't have anything to do with the ship actually needing to take damage.
And then you have someone like Lieutenant Blount: "When attacking, the defender is hit by your attack, even if he does not suffer any damage".
Throw 'Ion Pulse Missiles' on him ("If this attack hits, the defender suffers 1 damage and recieves 2 ion tokens. Then cancel all dice results") and all you need is the ability to fire the missiles on a ship. Even if they cancel all your hits and crits, the attack counts as "hitting" so you get to deal the 1 damage and 2 ions no matter what.
