If a character is strained AND his Willpower is strained AND he gets 2 stress and 2 fatigue, what happens?
Willpower, strained and insanity
His Fatigue goes up by 2, his Stress goes up by 2 and he draws a temporary Insanity card.

Ok, but the FAQ speaks only about a strained character, NOT a strained AND Willpower-stressed character.
Strained is the state of being Stressed and Fatigued.

Yes, but you can be Strained without having your Willpower Stressed.

It is the same effect however - as the Strained condition is a result of AT least one mental trait being stressed and one physical trait being fatigued - so additional stats entering the same condition do nothing additional other than stressing a second stat which adds die for each point over the stat. I hope this helps - it is an oddly phrased thing to try and explain.
I thought to be strained you needed fatigue over T and stress over WP ?
Nope,
if any physical characteristic is less than number of fatigue points, that characteristic is fatigued (misfortune die on tests)
if any mental characteristic is less than number of stress points, that characteristic is distressed (misfortune die on tests)
if WP is distressed then you are at risk of picking up insanity when you gain more stress
if a physical characteristic is fatigued when a mental characterisitic is distressed, then you are classed as strained. This is VERY BAD, you get insanity and both stress and fatigue make the insanity you gain worse.
Having willpower distressed when you are strained has no further impact, other than that already described above under the strained condition.
When strained you can also pick up multiple insanities in one encounter, bad, bad. bad...

The only other thing to add and what possibly causes confusion, is that you only pass out or fall unconscious when Fatigue exceeds 2x Toughness or Stress exceeds 2x WP. You can have more than 2x Strength in Fatigue or 2x Fellowship in Stress for example and you just keep racking up additional Misfortune dice on tests that use those Characteristics.
